DOCUMENTS REQUIRED FOR AIIMS CRE-5

At the time of Document Verification (original + one photocopy each):

1. Downloaded Registration Slip — of the online application form (self-attested)


2. Downloaded Admit Card — copy of CBT admit card


3. Date of Birth Certificate — 10th class marksheet or Birth Certificate showing DOB


4. Caste Certificate — if applying under SC / ST / OBC / EWS category, issued by competent authority:
   
   
   
   • OBC (NCL) certificate must be valid for financial year 2026–2027
   
   
   • EWS certificate must be valid as per Central Govt norms (OM dated 31.01.2019 format)
   
   
   
   


5. PwD / Disability Certificate — if applying under Persons with Disability category


6. Age Relaxation Certificate — from competent authority if claiming relaxation on grounds other than caste or PwD


7. No Objection Certificate (NOC) — mandatory if currently working in:
   
   
   
   • Central / State Government
   
   
   • Semi or Quasi Government
   
   
   • PSUs / Autonomous Bodies
   
   
   • Any institution funded by Government
   
   
   • Also required for contractual employees in above organizations
   
   
   • Must be uploaded online by 8 July 2026
   
   
   
   


8. Educational Qualification Certificates — all relevant degrees, diplomas, marksheets as per post applied


9. Experience Certificate(s) — from Head(s) of Organization/Department clearly mentioning:
   
   
   
   • Duration of employment (date, month, year)
   
   
   • Basic pay / consolidated pay
   
   
   • Nature of duties performed
   
   
   
   


10. Any other certificate as required by the respective participating institute

• Visit the official AIIMS Examinations website at www.aiimsexams.ac.in.


• Complete One Time Registration (OTR) if not already registered — a unique Candidate ID will be generated. Applications with duplicate or different Candidate IDs will be rejected.


• Log in using your Candidate ID and select "CRE-5" from the available examinations.


• Identify the group(s) for which you are eligible as per Annexure-I, and fill a separate application for each group you wish to apply for.


• Fill in all personal, educational, and contact details carefully. Ensure the information matches your original documents — no corrections will be allowed after specified dates.


• Upload clear scanned images of your photograph, signature, and left thumb impression strictly as per the 'Upload Image Instructions' given on the portal. Applications with incorrect/unclear images will be rejected.


• Upload your No Objection Certificate (NOC) if you are currently employed in any Government / Semi-Government / PSU / Autonomous Body. Failure to upload NOC by 8 July 2026 will result in disallowance from appearing in the examination.


• Pay the application fee — ₹3,000 for General/OBC, ₹2,400 for SC/ST/EWS, Nil for PwD — through Debit Card, Credit Card, or Net Banking. Pay separately for each group applied.


• Review the complete application thoroughly before final submission. Once submitted, corrections will not be entertained through email or letter.


• Download and keep a copy of the Registration Slip along with proof of payment for your records. No physical documents need to be sent to AIIMS.


• Regularly check www.aiimsexams.ac.in for updates on admit card, exam schedule, result, and choice/preference exercise notification.
